Reports to: All partners within the practice. Qualifications: Full registration with the General Medical Council Certification by the JCPTGP Registration on the Health Authority Supplementary List Effect and maintain membership of a recognised Medical Defence organisation approved by the practice, commensurate with the post holder’s duties and at their own expense throughout the period of employment. (The post holder is eligible to join the practice group scheme with the MDDUS . If the post holder uses the Practice Group scheme the surgery will cover the cost. If the post holder chooses to use their own scheme the post holder will be responsible for the fees) Personal Attributes: Committed to the delivery of high quality clinical care and the ability to work as part of a multi-disciplinary team. Salary & Hours: Salary Negotiable. Hours – 6 / 9 sessions Job Summary: The post holder will be required to provide personal medical services (PMS services) under the Personal Medical Services agreement between the NHS England and the practice, to patients of the practice by way of surgeries, clinics and relevant administrative work. Together with such other duties as are required of the practice in providing such services under the provision of the Primary Care Act 1997 and which are reasonably delegated to the post holder by the practice. Duties also include providing daytime on-call cover between the hours of 8.30am and 6.30pm as required on the days worked. Responsibilities The services provided by this post will include: • Responding to medical problems presented by patients on the practice medical list, including history taking, examination, diagnosis, treatment and referral where appropriate. • As well as undertaking face to face consultations the post holder will be expected to deliver Telephone Triage service. • Establishing appropriate systems to manage common chronic medical conditions such as asthma and diabetes. • Providing appropriate preventative health care and advice within the context of primary health care. • Attending primary health care team meetings and other meetings as may be necessary for your duties. • Auditing practice activity if requested. • Clinically related administrative and non-clinical duties needed for the delivery of Personal Medical Services. • Use of appropriate templates and codes as used for QOF and Enhanced Services. • The post holder is required to keep proper records of attendance, visits by and to any patients and all such other records as required by NHS legislation, or are reasonably required by the practice. This includes computerised records on the practice clinical system. • The post holder would be required to complete all paperwork and results relating to patients they have seen. They would also be required to complete paperwork including housing forms, child protection cases, holiday cancellation forms etc. You will not be required to complete PMA forms or examinations. • THE SURGERY HAS IMPLEMENTED A WORKFLOW PROCESS SO THE NUMBER OF HOSPITAL LETTERS REQUIRING ACTION BY A DOCTOR IS MUCH REDUCED AND UNLIKELY THAT MORE THEN 15 MINUTES PER DAY WILL BE SPENT ON THIS • Prescription of care including medication. • Duty doctor on-call responsibilities on a rota basis • To work at the branch surgeries if required • Attending surgery meetings when they coincide with working pattern • Providing a collaborative, supportive and holistic approach to patients of the practice. Personal and Professional Development.  Maintain valid full GMC registration.  The post holder will be expected to undertake necessary CPD activity as required for revalidation.  Continue professional development in agreement with the partners of the practice.  Undertake annual appraisal as stipulated by the GMC re-licensing regulations.  Undertake and internal Practice Appraisal on an annual basis.  Work within agreed clinical and professional boundaries and protocols of the practice;  Practice evidenced based health care consistent with current theory and practice; NB This job description is not meant to be exhaustive, and will be subject to regular review and amendment in the light of the development of the post.
